**Braden Scale** The Braden Scale is the most widely used risk assessment tool, evaluating six subscales: sensory perception, moisture, activity, mobility, nutrition, and friction/shear. Scores range from 6-23. Score of 18 or below indicates at-risk status; 12 or below indicates high risk. **Norton Scale** One of the original pressure injury risk tools, the Norton Scale assesses five factors: physical condition, mental condition, activity, mobility, and incontinence. Scores range from 5-20. Score of 14 or below indicates increased risk. **When to Assess** Perform risk assessment on admission, at regular intervals (every 24-48 hours for acute care, weekly for long-term care), and when condition changes significantly.
