Encourage ambulation and active range of motion. **Support Surfaces** Select surfaces based on risk level. Low-risk: high-density foam. Moderate-risk: alternating pressure or low-air-loss. High-risk: air-fluidized beds. Ensure heels are elevated using pillows or suspension devices. **Skin Care and Nutrition** Keep skin clean and dry while maintaining adequate hydration. Ensure protein intake of 1.25-1.5 g/kg/day with emphasis on Vitamin C and zinc. **Education and Documentation** Educate patients, families, and care team members. Document assessments, interventions, and outcomes. Conduct regular skin inspections.
